Everyday access
Support to reach the places you need to be, from appointments and shopping to study or work.
Practical, person-centred support for outings, appointments, hobbies, social connection and participation across Laidley and the Lockyer Valley.
Community access can be as practical as getting to an appointment or as personal as returning to a hobby, meeting people, shopping independently or trying something new.
Prismatic Care supports participants to take part in their local community in a way that feels comfortable and purposeful. The goal is not to fill time. It is to help you do more of what matters to you, with the level of support that suits you.
